Epinephrine in LA (Adrenalin®)

Presentation

Epinephrine is a an alpha-/beta-agonist that is administered as an adjuvant in local anesthetic cartridges.

Epinephrine is also used as an emergency drug for treatment of anaphylactic reaction and a vasoconstrictor to decrease systemic absorption of local anesthetics and to increase the duration of anesthetic action. The use of Epinephrine may decrease superficial hemorrhage.

    Hi I have a question so when using LA with epinephrine, what figure will we rely on max safe dose of 0.2mg or 0.003/kg (e.g when calculation Lidocaine 2% p with 1:100000 epi for a 50 kg patient. Can I get the reference for .003/kg for epinephrine as previously I studied in the articles about only 0.2 mg as max safe dose of Epinephrine. Please elaborate this statement

    “For adults and children > 12 years old, 3 mcg (0.003 mg) of epinephrine/kg of body weight or 0.2 mg epinephrine per dental appointment is usually the maximum recommended dose.”

    You have mentioned typical maximum recommended dose for adults treated with lidocaine 2% with epinephrine 1:100,000 is 11 cartridges, however as per Prof D.Haas’s article(An Update on Local Anesthetics in Dentistry) Max Recommended dose is 13 cartridges.

    Could you please clarify what to follow?
