Your Opinion: what is the etiology behind the erosion on the 7-4 and how would you treat it?

I am completely stumped as to the etiology behind the extreme wear/erosion on this child’s 7-4. The child is 9 years old and has generalized wear and early erosive-looking lesions on his occlusal/incisal surface.
