Managing Deep Cervical Margins

Managing a Class II restoration with a deep cervical margin is a challenge that most dentists have faced at one time or another. And even though it may seem like a routine filling to the patient, for the clinician it can be a stressful procedure.

In this episode of CDA Oasis Live, Dr. John O’Keefe, Director of Knowledge Networks CDA, invites Dr. Sunny Virdi, general practitioner from Winnipeg MA, to walk through the process in this visual presentation from his own practice. Dr. Virdi discusses the limitations of composite materials, he illustrates the matrix + matrix technique, and offers other useful tips on achieving a successful outcome.

  • All dental materials have their limits, even composite. When we have deep interproximal decay it is important to take a step back and assess whether the material is a good choice or not.
  • A key factor is whether or not you have a sliver of enamel in the cervical box. If you have enamel at the base of the box then a direct restoration will work well.
  • Matrix + Matrix technique: in order to get a better seal, trim a sectional matrix and place inside the matrix.
  • Consider a small laser gingivectomy to expose some of the margins so that the rubber dam will sit better.
