Follow-up: Anti-infectives pediatric dosage

This is a follow-up post to the Refresher: Why use anti-infectives?

A number of dentists inquired about the pediatric doses for the anti-infectives mentioned in the post. Sources of information: Lexicomp and e-Therapeutics.ca

Amoxicillin

Amoxicillin and Clavulanate

Cephalexin

Calrithromycin

Sinusitis, bronchitis, skin infections: Oral: 7.5 mg/kg/dose every 12 hours for 10 days

Clindamycin

Infants and Children:

Doxycycline

Usual dosage range:

Erythromycin

Usual dosage range: Infants and Children:

Oral:

Metronidazole

Children ≥1 month of age: 15–35 mg/kg/day po in 3 divided doses.

Penicillin V Potassium

Nystatin Oral Suspension: Oral candidiasis

Fluconazole

Usual dosage range: Oral, I.V: Loading dose: 6-12 mg/kg/dose; maintenance: 3-12 mg/kg/dose once daily; duration and dosage depend on location and severity of infection

Itraconazole

Candidiasis:

Ketoconazole

Fungal infections: Children ≥2 years: Oral: 3.3-6.6 mg/kg once daily
